The Pennsylvania School Health Code requires that all students receive a physical examination upon original entry (Kindergarten or grade 1) and when entering grades 6 and 11. Physical exams done after August 20, 2016 will meet the requirement for the 2017-2018 school year. Physical exam forms are sent home with Fifth graders in the fall and spring to prepare for the required Sixth grade physical.
Physical exam forms are due by August 25, 2017.
Doctors' schedules can fill up fast, so call now to for an appointment for your Sixth grade physical exam. Be sure to take along a copy of the physical exam form.
For your convenience, you may arrange to have the completed form faxed to school to the attention of the School Nurse. Our fax number is 717-382-4786.
Students who do not return a completed physical exam form may be examined by the school physician with parents' written permission. Call the Health Room
